Loan Application
Use the drop-down menu to select which type of loan you are applying for.

Home Equity Line of Credit
Be sure to select "Home Equity Line of Credit" in the drop-down menu.

Debit & ATM Card
Please print the credit card application, fill it out, and either drop it off or mail it to the address shown.

HFS Online Banking & e-Statements
Please print the HFS/e-statement application, fill it out, and either drop it off or mail it to the address shown.  If you already have HFS access and wish to opt in to e-statements, simply log into your account, click on statements, then "Opt In".

These forms are intended to make it simple and easy to maintain your Citizens account and keep your information up to date.  If there is a form you need that is not on the web, please call (269) 381-1938.

Change Address & Phone Number
Please print the "Change Address & Phone Number" form, fill it out, and either drop it off or mail it to the address shown.

Account Switch Kit
The following three forms comprise the "Account Switch Kit".  Please use these to simplify the process of switching important aspects of your account to Citizens Credit Union.

• Direct Deposit Authorization
  Complete this form for each company you have direct deposit with.  For your payroll deposit, please give this form to your Human Resources/Payroll department. If you have social security or other governmental direct deposit, please be sure to obtain the correct forms from Citizens Credit Union.
  
• Automatic Payment Switch
  Complete and sign one copy of this form for each automatic payment and mail them to the merchants you currently authorize to take automatic payments from your account. This form will notify merchants that you wish to have your automatic payment transaction redirected to Citizens Credit Union.
  
• Account Closing
  Complete this form and return it to the financial institution where the account you would like to close is located. One form should be used for each account.  Please do not use this form to close an account at Citizens Credit Union.
